Note should be taken that this application is subject to the following conditions:
1. The client must submit: an authentic Namibian identification document; valid proof of permanent residence and a valid working permit, copies of the lease agreement and proof of registration with the Ministry of Trade and Industry,
2. The client must obtain a consent letter/town planning certificate from Town Planning Division; proof of compliance with building requirements from Building Control Division; complete the Industrial Effluent Discharge Permit application form from Bulk and Waste Water Division (only applicable to butcheries, dry cleaners/laundry and factories); proof of compliance with the Fire Safety Requirements from Emergency Management Division; proof of compliance with Health Requirements from Health Services Division.
3. The client must attach medical certificates for food handlers (only applicable to food preparation premises);
4. The client must specify the type of business activities to be conducted by the business.
